The Public Transport Authority said two railcar bogies, structures which support the weight of a train, partially came off the tracks while departing Bayswater station at low speed.
“The train has come to a stop with all passengers safely evacuated,” a PTA spokesperson told 7NEWS.com.au in a statement.
“The PTA is investigating the cause of the incident.”
Trains on the Airport, Ellenbrook and Midland lines have been cancelled between Perth and Redcliffe, Perth to Morley and Perth to Bassendean.
“This is also causing delays on the Fremantle Line,” Transperth said in a disruption update on Monday afternoon.
“Train replacement buses are being organised.
“Please allow extra travelling time and be aware that replacement buses may not connect with timetabled bus and train services.”

Transperth staff have been directing workers leaving the city to waiting bus services.
“It’s really hot, I’m really hot,” one person told 7NEWS.
Another said travellers on public transport would have to show “patience”.
A third said they were met with a long queue and would struggle to pick up their kids from sport on time.
Commuters have been told it is unclear when the issue will be corrected.
